TOPIC: Imaging TYPE: Fellow Case Reports INTRODUCTION: Radiation pneumonitis (RP) usually occurs 3-6 months after radiation and fibrosis within 6-12 months. We report a patient who developed the classic imaging features of RP 2 years 6 initiation chemotherapeutic agent – Paclitaxel, consistent with phenomenon recall (RRP). RRP can occur when agents such as doxorubicin, erlotinib, etoposide, gemcitabine, paclitaxel, pemetrexed immune checkpoint inhibitors are administered to has received prior therapy lung.1 Though rare, its recognition will allow for discontinuation offending use immunosuppressive drugs like steroids in an attempt reverse process. CASE PRESENTATION: A 73 year old woman was diagnosed stage 4 lung cancer treated full course chest radiotherapy chemotherapy. Following failure respond multiple agents, started on paclitaxel initial diagnosis. admitted fever respiratory distress. CT Chest (See Fig.) revealed consolidation which strictly confined portal that given two ago indicative RP. done 1 completion showed no changes. Hence, diagnosis made, stopped improved steroids. DISCUSSION: is form injury previously irradiated parenchyma chemotherapy or immunotherapy beyond time frame typical pneumonitis. Exact mechanism unclear. One hypothesis indicates administration systemic anti-tumor triggers “remembered” reaction remaining surviving cells areas resulting enhanced sensitivity drugs. leading heritable mutation produces subgroup defective stem anti tumor alternate possibility.2 Another explanation offered leads local vascular permeability proliferative changes decrease threshold subsequent from antitumor immunotherapeutic CONCLUSIONS: While there clear consensus diagnosis, should be suspected seen outside conventional patients receiving these agents. Recognition this entity important order discontinue provide potentially beneficial steroid. Reference #1: Hochstrasser et al. Interstitial treatment pemetrexed: rare event? Predictive markers of radiation pneumonitis.

Radiation pneumonitis is an acute-phase response to radiation therapy and a common complication that affects a patient's quality of life. Under the need to reduce the incidence and severity of radiation-induced pulmonary complications as well as to identify patients at risk, several investigations on potential predictive markers of radiation pneumonitis have been conducted. The present study re...

Radiation recall pneumonitis induced by chemotherapy after thoracic radiotherapy for lung cancer

BACKGROUND Radiation recall pneumonitis (RRP) describes a rare reaction in previously irradiated area of pulmonary tissue after application of triggering agents. RRP remains loosely characterized and poorly understood since it has so far only been depicted in 8 cases in the literature. The objective of the study is to disclose the general characteristics of RRP induced by chemotherapy after tho...

Histopathological Studies on Radiation Pneumonitis.

Studies on radiation pneumonitis have been made from the beginning of the 20th century in Europe and America. Prior to the Second World War, many articles have appeared including the works of Engelstad5> and Warren et al.25) who described the general aspects of this problem. In Japan, the incidence of this disease has been rare. A case report by Hamaguchi and Akazaki9) was only available before...

Curcumin Mitigates Radiation-induced Lung Pneumonitis and Fibrosis in Rats

Radiation-induced lung injury is one of the most prominent factors that interfere with chest cancer radiotherapy, and poses a great threat to patients exposed to total body irradiation. Upregulation of pro-oxidant enzymes is one of the main mechanisms through which the late effects of ionizing radiation on lung injury can be exerted. Interleukin (IL)-4 and IL-13 are two important cytokines that...

Teaching Case of the Month Radiation Pneumonitis

Radiation-induced lung injury is a serious complication following thoracic irradiation for the treatment of lung, breast, or hematologic malignancies. Two distinct lung injury patterns have been described: early radiation pneumonitis and late radiation fibrosis. Both syndromes commonly include dyspnea and cough. The chest radiograph invariably shows pulmonary infiltrates. The following case des...
